Template Practice Resources

The below resources are available to help reach parents throughout the year about getting their children (especially any child with asthma) vaccinated against influenza.

The following tools can be tailored for use within a variety of practice settings to help support immunization efforts targeting pediatric asthma patients (or other children at risk) in time for the influenza season.

On Hold Script
This script can be customized and recorded as the message parents hear when they are on hold to schedule an appointment or speak with office or clinical staff.  This reinforces the need for annual influenza vaccination and gives instructions for how to get their child vaccinated (e.g., schedule an appointment, attend an immunization clinic, etc.).

Office Closed Script
Your practice may have an existing office-closed recording, which can highlight important influenza prevention messages, as appropriate, during the year.  This suggested script can be incorporated within the office’s main default prompt, urging parents to help protect their children by scheduling an annual influenza vaccination appointment or bringing their children to an upcoming immunization clinic.

Reminder Letter
The template reminder letter can be customized and mailed to parents of children with asthma, alerting them to the importance of annual influenza vaccination.  Space has been provided to insert information relevant to vaccine clinics and/or extended office hours.

Reminder E-mail
This suggested e-mail can be tailored and distributed electronically to parents of children with asthma, educating about the importance of annual influenza vaccination.  Space has been provided to include details regarding your practice’s vaccination program.

Reminder Postcard
This postcard can be mailed as a reminder to parents about the upcoming influenza season and the need to schedule vaccinations for their children – especially if they have asthma.  Due to HIPAA regulations, this postcard is not personalized.  

The language can be cut and pasted onto a stock-paper postcard or incorporated into a pre-existing mailing and distributed to patients, reminding them about the importance of influenza vaccination.
